The full set of more than 65 new data collection tools to measure the health-related outcomes of community development activities. The Success Measures Health Outcome Tools will help both community development and community benefit practitioners document and demonstrate the impact of their efforts on individual and community health. The tools measure both primary and secondary data through many formats, such as surveys, observations, interviews and tracking, and focus on traditionally hard to measure areas, such as behaviors, attitudes and influences.
