The Success Measures Health Outcome Tools have just been released as a valuable new free resource for the field. The new tools were developed to help evaluate and document the health-related outcomes of a wide range of affordable housing, neighborhood revitalization, workforce development, supportive service and community engagement programs. The suite of more than 65 tools focus on previously hard-to-measure areas, such as changes in individual and community health status, attitudes, behaviors and beliefs.
Organizations are welcome to use the tools “as-is” or to download and modify them to align with your community work and the focus of your evaluation.
