Bon Secours Mercy Health (BSMH), a large regional health care system, has been investing in the social determinants of health in all its markets. Looking to ensure that they understand the results and impact of these investments on communities, BSMH engaged Success Measures as its evaluation partner.
Success Measures developed a theory of change, evaluation plan and measurement framework for BSMH’s investment pool for community health, with targeted evaluations specifically for two of its funds. New analysis and communication tools including reporting and application templates and an evaluation process will support documenting outcomes to align with the theory of change. With guidance from Success Measures evaluation plan, BSMH is moving forward to better assess their work across the portfolio. (2023)
