Health Outcomes

More than 65 new data collection instruments are now available to measure the health-related outcomes of community development and community benefits programs. The Success Measures Health Tools are available at no cost in a new online publication and can be used to measure changes in health status, attitudes, behaviors and beliefs at the individual and community levels.  Availability, quality and cultural sensitivity of health care providers and practices are also covered in the tools.
