Health Outcomes
More than 65 new data collection instruments are available to measure the health-related outcomes of community development and community benefits programs. The full set of health tools are available in PDF format at no cost in Success Measures Evaluation Tools for Community Development and Health. The tools can be used to measure changes in health status, attitudes, behaviors and beliefs at the individual and community levels. Availability, quality and cultural sensitivity of health care providers and practices are also covered in the tools.
